I-75 south to Rte. 725 (Centerville-Miamisburg Road) - Exit at Rte. 725
Turn east (left) on Rte. 725
4 miles to center of Centerville, where Rte. 725 becomes Franklin St. - continue straight east on Franklin St. CHS is located 1/2 mile east of the center of town on the south (right) side of Franklin St.
